The 7 Questions Dermatologists Get Asked All The Time – And Their Answers

From acne and rosacea to simply not knowing where to start with a good skincare routine, we turn to dermatologists for all manner of skincare concerns. Given that they have seen, done and experienced it all when it comes to the skin, what are the questions they get asked most often? And what advice do they give? British Vogue sat down with three experts to find out.

Does your diet affect acne?

For the vast majority of people, acne purely comes down to your hormones and genetics,” says Dr Anjali Mahto. “That said, there is a small, select group of people that may be sensitive to dairy and refined sugars. I don’t recommend people cut things out of their diet at random because I think that can lead to issues around food restriction and disordered eating. But if you are noticing that your skin is breaking out when you eat dairy – and I’m not talking about a splash in your coffee, but huge amounts or taking whey protein supplements – there is probably some benefit in switching to a plant-based alternative that’s got a low GI index. Think unsweetened soy milk or almond milk, which are both better than oat milk.” Dr Justine Kluk agrees, stating that dietary changes alone are not enough to control acne. “They can form part of the management approach alongside prescription treatment, but don’t replace it in most cases,” she says.

Is the SPF in moisturiser equivalent to the one in sunscreen?

The SPF in your moisturiser is tested the same way as an SPF in sunscreen, so an SPF 30 moisturiser should provide an SPF of 30,” explains Dr Justine Kluk. “The main issue is that these formulas are less likely to be rub and water resistant and may be applied a lot more thinly than sunscreen. It’s for this reason that they may not offer the same level of protection. It is also worth noting that moisturisers containing an SPF may not contain any UVA protection and, as a result, will not protect against UV ageing.”

Do collagen supplements actually work?

If you look at the majority of data, at this moment in time, there isn’t any really good evidence that collagen supplements actually work,” says Dr Mahto. “That’s essentially because collagen is a protein – just like eating a piece of steak or tofu is protein. All that will happen is your gut breaks it down into constituent amino acids and doesn’t think, ‘I need to send it to the skin’, so it gets passed around the body. Also, if you’ve got enough expendable cash to be buying collagen supplements – they’re not cheap – you’re probably also the kind of person who is wearing sunscreen and following a good, healthy diet and using a retinol, so it becomes tricky to figure out whether it’s the collagen supplement working or the other things. I’m sceptical, but if you can show me good data that works, I’m willing to change my position on it.

Will my breakouts ever go away?

They might, but the reality for many women is that they often continue into the thirties and beyond,” says Dr Sam Bunting. “The good news is that the right anti-acne skincare routine will often be a highly effective plan for tackling premature ageing too, so your skin may well look better and better as time goes on.

What is the right age to start having injectables?

Lots of people ask if they’re too young or too old for injectables,” says Dr Mahto. “Generally, people that are in their mid-thirties onwards have figured out whether it’s a reasonable time to get started. Usually what I say in this scenario, is that it’s not actually about your age, it’s more about how your skin is ageing. That depends on your individual genetics – how your parents age – as well as how much sun exposure you’ve had, your diet, how stressed out you are and how you sleep. You can have somebody in their late twenties who’s had very little sun and their skin is ageing beautifully and they don’t need any injectable treatments. On the other hand, you could have a 28-year-old who has a really expressive face, they’ve enjoyed sunny holidays and outdoor sport, and they’re starting to get lines when their forehead is at rest or noticing a loss of volume in fat in their face. For someone like that, it might be a reasonable time to start. Different ethnic groups also age differently – somebody who has really fair skin, blonde hair and blue eyes will start to get wrinkles more quickly than somebody who has Asian or Black skin, because their melanin will protect them. The flip-side is that those with Asian and Black skin tend to start losing volume in their faces more quickly, so they tend to need filler before they need Botox.”

Is my skincare routine working?

I think there is still a basic lack of understanding around what you actually need in a skincare routine and what you don’t,” Dr Mahto says. “People buy into the buzz about the latest ingredients – whether that’s niacinamide or tranexamic acid – but actually the average person does not need to be using every single one of those ingredients. What you’re trying to do is use as little as possible on your skin, and to look for ingredients that target multiple things. Vitamin A is anti-ageing, good for acne and pigmentation – so why use niacinamide and tranexamic acid and retinol when you’ve got one ingredient that will do a really good job of that? I spend a lot of time stripping back people’s routines, rather than adding things in.” 

Will anyone be able to tell if I’ve had Botox?

Not if it’s done well,” points out Dr Bunting. “I talk to patients about softening strong expression lines and releasing the tension from the face nowadays – it’s a far cry from the frozen faces of the ’90s. Microdosing means no one else ever has to know.”

Why The Internet Can’t Stop Raving About Tranexmic Acid

The brightening skincare ingredient can tackle hyperpigmentation.

As far as brightening skincare ingredients go, vitamin C is an A-list star. However, there are other effective ingredients that can treat post-inflammatory hyperpigmentation from acne, sun damage, or melasma that deserve some time in the spotlight, too. 

Tranexamic acid is one such ingredient having a moment, with more and more skincare influencers and beauty brands shining light on its benefits and including it in product formulas. While tranexamic acid can be effective on its own, it works even better as an ensemble with other dark spot-fighting ingredients such as vitamin C, kojic acid, niacinamide, and more.

What Is Tranexamic Acid? 

“Tranexamic acid is a synthetic form of lysine, which is an amino acid needed to make proteins,” says Dr. Shari Marchbein, board-certified dermatologist in NYC. “It works by decreasing the production of melanin and we know that the oral form is much more effective at treating melasma than topical form. That being said, serums and other products that contain this ingredient have a lot of potential to help improve hyperpigmentation.” 

The ingredient originally was used as a hemostatic agent to help blood clots, but recently has been utilized as a brightening ingredient to help minimize hyperpigmentation as well as melasma.

What Are the Benefits of Using Tranexamic Acid? 

One of the major benefits of tranexamic acid is that it plays nice with other brightening ingredients, so you can really zero in on hyperpigmentation. 

“There are many treatments for dark spots and these often work well together including licorice, niacinamide, kojic acid, tranexemic acid, retinoids, chemical exfoliants [such as glycolic acid, an alpha-hydroxy acid], and more,” Dr. Marchbein says. The dermatologist often recommends serums with tranexamic acid and other brightening agents be used in the same routine for the ingredients to work synergistically to improve post-inflammatory hyperpigmentation and melasma. 

Tranexamic acid is also a safer, effective alternative to hydroquinone, a potentially irritating bleaching ingredient. “There aren’t many options when it comes to safe, effective skin lightening bioactives,” says Krupa Koestline, clean cosmetic chemist and founder of KKT Consultants. “Hydroquinone is banned in the EU and restricted in many countries due to its safety concerns. Tranexamic acid has shown promising evidence as a plasmin inhibitor and therefore an effective treatment for UV induced discoloration, dark spots, and redness.”  

What Are the Side Effects of Tranexamic Acid? 

All skin types can use tranexamic acid, but like adding any other new ingredient to your skincare routine, it’s best to do a patch test to ensure you won’t experience irritation. 

It’s also important to wear SPF when using tranexamic acid, along with other brightening ingredients, because the sun can make hyperpigmentation darker. 

“Remember that before you spend your money on antioxidant serums, brightening ingredients, and retinoids to improve the tone of your skin and hyperpigmentation, the most important and first step is diligent daily sun protection,” Dr. Marchbein says. “It is key to reduce the appearance of brown spots (otherwise you are literally throwing your money away).” The dermatologist recommends a broad-spectrum SPF 30 or higher should be applied to the face, neck, and ears every day — even in the winter.

How Do You Add Tranexamic Acid to Your Skincare Routine? 

Dr. Marchbein says to use tranexamic acid once or twice a day. “I also layer tranexamic acid containing serums over Vitamin C serums and under SPF in the morning and under retinoids at nighttime, so this can safely and effectively be combined with multiple other actives.”

The active can be found in serums, moisturizers, and toners, so it’s entirely up to you what step of your routine in which you want to incorporate it. 

That being said, Koestline says serums are a popular way to go. “Most people do like using actives in their serum layer since you’re applying it before other products.”

12 Korean Beauty Trends To Try In 2021

Snail mucin. Bee venom. Glass skin. These are just some of the beauty trends to emerge from South Korea in the past five years. Whether you’ve dabbled in a bit of donkey milk (good for rejuvenating the skin with protein and fatty acids) or you’ve played it safe with a weekly face mask, K-beauty is everywhere. In fact, Allied Market Research says that by 2026, the K-beauty market will be worth an estimated $21 billion. According to Jenni Middleton, director of beauty at trend forecasting company WGSN, “During the coronavirus pandemic, consumers searched more for K-beauty, looking for innovative products to add to their lockdown beauty regimes.”

Like most cultural phenomena, K-beauty is ever-changing—what was big last year may not be as popular this year. As Middleton observes, we’re seeing the traditional 10-step routine give way to a more minimalist approach as conscious consumers react against fast fashion and excessive packaging. Elsewhere, playful gimmicks such as color-changing effects or jellylike substances are being passed over in favor of science-backed formulas.

1. Hanbang ingredients

Hanbang ingredients are traditional herbal ingredients used in Korean medicines and they’ve long been a staple in Korean life. For example, ginseng root, houttuynia cordata, sacred lotus, and rehmannia boast antiaging, anti-inflammation, and regenerative properties.

2. Acid layering

K-beauty has been incorporating more acid into its products, but with a gentle approach that focuses on striking the balance: Too much can irritate and aggravate your skin, too little will yield no results, so products with an optimal amount is key. Use the right balance of AHAs and BHAs (plant and animal-derived acids) to gently exfoliate dead skin cells and smooth skin texture.

3. Carrot seed oil

Carrot seed oil is an unsung hero at the moment, although it has been used in K-beauty for more than 10 years. It contains vitamin A and is a great antioxidant. It’s antiaging, antifungal, antibacterial, and anti-inflammatory—so it’s ideal for anyone looking to brighten up their skin.

4. Gentle retinol

K-beauty’s ‘skin first’ approach will continue through 2021, especially given that self-care and skin care are so important right now. There’s no denying retinol’s powerful antiaging properties, but the K-beauty approach uses a lower percentage, so the skin stays healthier and less irritated. Retinol is highly efficacious without causing unnecessary damage.

5. Centella asiatica

[The year] 2021 is less about what’s ‘buzzy’ and more about what’s tried-and-true, with a focus on calming the skin. Centella asiatica [an herb grown in Asia, known for being anti-inflammatory]—or ‘cica’—is huge right now. With everyone dealing with the prolonged stress of the pandemic and dreaded ‘maskne,’ soothing irritated, angry skin seems to be at the forefront of people’s minds. Cica is the ingredient that everyone wants to add to their routine.

6. Clean beauty

More brands are developing products free of chemical additives, artificial ingredients, and fragrance. Products will be even more gentle with effective plant-based ingredients, and many brands are becoming vegan as well. Consumers are more aware of what they put on their skin.

7. Pre-, pro-, and postbiotics

This year, inner and outer wellness brands and products will gain more popularity. For example, brands that focus on pre-, pro-, and postbiotics; microbiome-friendly skin care; and consumable supplements, which benefit both the skin and the gut.

“K-beauty will shift more towards a holistic approach, linking skin care and internal health. I take probiotic supplements for my bouts of eczema and I love using K-beauty products with fermented ingredients. I regularly use 107—it uses aged [seven- and 10-year-old] vinegar [that promotes good gut health]. Their vinegar tastes delicious with honey!”

8. Flexible minimalism

A few years back, we were oversaturated with the ‘10-step Korean skin-care routine.’ The ‘skin-care diet’ [using fewer products and steps] that followed was a pushback against that, but it was too restrictive for those who wanted more results than could be attained with just the basics.

Flexible minimalism is a focus on clean and simple product lines, which makes customizing your routine easier. There will also be a push towards pared-back lists of ingredients. Single and minimal ingredients are appealing because of their simplicity and high concentration of the hero component.

9. At-home indulgences

Skin care has a functional element—it has to work and deliver results—but I expect products that provide meditative, soothing, and spa-like moments to take off in a big way. They can transport you mentally and emotionally to another headspace.

10. Hyphenate and hybrid skin care

We’ve started seeing ‘skipcare’ as a K-beauty trend, where the focus is on a pared-down, simple, and minimalist routine. We will be seeing more efficient and effective multitasking and versatile products—what we like to call ‘hyphenates’ or ‘hybrid’ skin care.

11. Skin detoxifying and barrier strengthening

“The belief that ‘skin is a reflection of your mental state’ comes from Korea, and growing up, my mother emphasized this to me many times. We’ll see more barrier-strengthening ingredients that boost immunity, such as mushrooms, plus detoxifying herbs including mugwort and ginger. Ceramides [which form a protective layer to help prevent moisture loss and visible skin damage] will make a comeback too.”

12. A boost in body care

In Korea, many body-care rituals originate from the bathhouse culture, where milk treatments are slathered on the face and body, and baths are steeped with skin-beneficial ingredients, such as green tea and probiotics. During a difficult year, personal self-care has taken on new importance for many, so we expect to see the definition to include all of the skin, from head to toe.

7 Simple Winter Skincare Rules To Put Into Practice Now

Ah yes, it’s winter again. Forget your bones, you can probably feel it on your face, now home to dry, flaky skin. Seeking solace in a favourite face oil or moisturiser might seem like the only answer (and they can help, more on this later), but there are a number of other things to be aware of when it comes to your winter skincare regime. If you refuse to let your skin suffer as a result of plummeting temperatures this year, read British Vogue’s seven rules of winter skincare – they’re simpler than you might think.

Keep your skin barrier strong

“As we move into winter, our skin is exposed to variations in temperature and humidity, as well as wind and rain, which can place stress on our delicate skin barrier. It’s the perfect time to rethink your skincare routine to battle environmental stresses,” explains consultant dermatologist Dr Thivi Maruthappu. The key indicators of skin barrier disruption are tight, irritated, itchy, and dehydrated skin.

Even in the months when the weather is less temperamental, our skin barrier is subject to disruption – excess use of stripping skincare products and external aggressors like pollution can all affect it – but it’s especially important it’s looked after in winter. Look for skincare that contains ingredients like niacinamide (try Paula’s Choice Clinical 20% Niacinamide Treatment), which “increases ceramide production in the skin, is anti-inflammatory and fights uneven pigmentation”, explains Maruthappu, as well as ceramides themselves (check out CeraVe), lipids, and richer creams that lock moisture in.

Medik8’s new H.E.O. Mask is exactly the tonic for winter skin, as it contains humectants, emollients and occlusives in optimal ratios, to first deeply hydrate, and then lock in moisture. Use it once or twice a week to tackle dehydration and dryness. Maruthappu is also keen to point out that upping your intake of healthy fats helps moisturise the skin from within – look to her Instagram page for sources of barrier-boosting fatty acids. “Look after your skin barrier and it looks after you,” she says simply.

Nail your nighttime regime

It’s at night that our skin goes into repair and restore mode, so it’s key to get your evening skincare routine in check. Facialist Debbie Thomas recommends cleansing with a non-drying acid cleanser – “look for polyhydroxy acids (PHAs), as they are the kinder cousins of alpha hydroxy acids (AHAs)” – like Exuviance’s Gentle Cream Cleanser, and then following up with an active product. “I alternate retinol with peptides, which are the second most proven ingredient when it comes to skin health and regeneration after retinol, and then apply a ceramide-rich hydrator to seal in the actives and protect the skin,” she explains.

Thomas is quick to warn about retinol, however, and says that though you might assume winter is the best time to start using it, the skin is already prone to becoming irritated and dry in the cooler months, so it’s important to tread carefully. “It can take several weeks for the skin to acclimatise to retinol use – it’s common to experience some dryness and redness – so if your skin already goes this way in winter, the combination of both could be unbearable and difficult to deal with. My main advice is not to overdo it.” Those already using retinol can continue as normal.

Dial down the exfoliation

When flakes strike, sometimes it feels like the only route is to exfoliate them away. Actually, this can further impair the skin barrier, leading to more skin issues. “I tend to advise reducing the frequency of exfoliation to once or twice a week,” says Maruthappu, “And avoid combining physical exfoliants, like grainy scrubs, with chemical exfoliants, like alpha or beta hydroxy acids, as this can lead to redness and irritation – particularly if you are also using a retinoid product.” The secret? Don’t overdo it with your skincare – less (and gentle) is more.

Load up on antioxidants

One of the biggest challenges for our skin in winter is the constant changes in temperature – moving from the heat to the cold outside wreaks havoc on our skin. Spending time inside with less fresh air also has its issues: “Recycled air has more toxins in it and central heating removes water from the atmosphere, which in turn removes water from the skin,” explains Thomas, who is a big fan of keeping an air purifier in the room you spend the most time in to promote healthy skin.

Antioxidant-rich skincare is also important, as it helps defend the skin against micro-toxins caused by recycled air, as well as those from pollution, UV and blue light damage, all of which are very much real, even in the depths of winter. Look for ingredients like vitamin C, vitamin E, resveratrol and niacinamide.

Avoid oils if you’re oily

Don’t assume that the cold months mean you have to switch your favourite moisturisers for face oils. While drier skin types can benefit, oilier ones should steer clear. “I generally recommend face oils for those with dry skin, as oils tend to sit on the skin surface and prevent further moisture loss,” says Maruthappu. “But the added benefit of a separate moisturiser can help to moisturise deeper layers of the skin. I tend to advise against oils in oily or acne-prone skin, as this can trigger breakouts by causing further congestion.” Those with oily skins should instead stick to non-comedogenic formulas that contain ingredients like dimethicone, ceramides or hyaluronic acid.

Heavier isn’t necessarily better

Just as with oils, thick and heavy formulas aren’t always best for the skin – although they do have their place in some skincare regimes. Thick, nourishing balm cleansers are a wonderful way to treat skin to some pamper time – try Chantecaille Rose De Mai Cleansing Balm – but they won’t necessarily hydrate skin. “If you apply a lot of heavy products to the surface, your skin’s sensors read this as not requiring true hydration, so they won’t absorb the required water into the deeper layers of skin,” explains Thomas. “After a time, the deeper layers become lazy and unhealthy, which eventually means more dryness and more irritation on the upper layers.” To remedy this, look to lots of hydrating ingredients like hyaluronic acid (a popular one is Oskia’s Isotonic Hydra Serum), and squalane, and simply seal them in with good hydrators, as mentioned earlier. “The best way to hydrate your skin is from within, so drink lots of water too,” advises Thomas.

Vitamin D supplements are a must

If you’re already an avid British Vogue reader, you’ll know the importance of taking a vitamin D supplement in winter; most in the UK aren’t getting enough year-round, let alone in the colder months when the days are shorter and darker. It’s important for our skin, too. “Vitamin D is key for the skin’s defences,” says Thomas. “Inflammatory conditions, like acne, rosacea, and eczema often flare up when we are deficient in it.” On top of that, a lack of it can negatively affect our mood, causing further hormonal imbalances, and meaning our skin is infinitely more likely to misbehave.

Do Collagen Supplements Actually Work?

One of the building blocks of healthy skin (and a number of other parts of our body, like hair, nails and bones), collagen plays a key role in healthy, smooth and supple skin. Disappointingly for us, however, optimum levels don’t last, and our body’s ability to produce it begins to drop from our mid twenties on – prompting a loss of elasticity, firmness, radiance, and that plumpness that we’re all so keen to cling onto as we get older. “We can’t prevent it, but we can slow it down,” says cosmetic surgeon Dr Benji Dhillon. Yes, applying vitamin C and SPF is important, but experts now agree that taking a collagen supplement could help, too.

“A collagen supplement delivers various types of collagen protein when taken daily, in either a powder, gel, drink, capsule or tablet format,” explains Kay Ali, official nutritionist at Beauty Pie. “While 28 different types have been identified, type one collagen is most commonly found in skin, bone, teeth, tendons, ligaments, vascular ligature and organs. It’s typically extracted from marine, porcine or bovine sources.”

Taken daily, clinical trials have shown a collagen supplement has the ability to slow down the body’s breaking down of its own collagen. In fact, the marine collagen in Beauty Pie’s soon-to-launch Collagen Super Powder reduced it by a huge 31 per cent. “With daily ingestion at an effective dose, you can expect to see plumper and more dewy-looking skin,” says Ali. Do Beauty Supplements Really Work?

There are 134 search results for “beauty supplements” on Sephora.com alone, I can’t even imagine what the growing number is these days besides just Sephora. Many of these companies take advantage of the fact that many peoples’ insecurities are based on thoughts that they don’t have clear skin, strong hair and nails, enough collagen, etc.

The descriptions are usually pretty similar across the board, the promise is:

“Formulated by doctors, these supplements help nourish, firm, brighten the skin, while combatting visible signs of aging like fine lines and pigmentation…” you get the drift.

In reality though, mostly all of them are NOT made by doctors (in fact doctors recommend against them), and the high price tag (leading you to believe that you’re getting wonderful ingredients that’ll work right away) is simply a rip-off! For example, Algenist is advertising their Chlorella and Spirulina lines at $65 for a small amount, when you can get these ingredients in a grocery store. And sure, they might mention that their is “ethically sourced” or some big claim that there is no way for you to verify.

Sure, some people claim to notice a difference, but more often than not, the difference is very minimal, like softer skin, brighter complexion, and thicker eyebrows, which can be achieved through a proper diet and a skincare routine anyway!

A huge factor to note is the Placebo Effect – when you believe you’re in fact getting those benefits, you’ll convince yourself that the supplement is working, and in turn keep repurchasing it. It only adds to the problem when the shape of the vitamins is a cute gummy bear, or an extremely-sugary pill.

Collagen supplements are one of the most popular ones: collagen is in everything nowadays, even topically applied collagen has made its way into the market. However, there’s still no research the supports the fact that ingesting or applying collagen to your face or body does anything. The only studied support for external collagen applies to any treatments you might be getting (laser, etc) and engaging collagen with it while the skin is repairing, but that’s it.

Not to mention that the U.S. Food and Drug Administration (FDA) doesn’t regulate supplements the way it does drugs. So you never really know what you’re getting when you buy one of these products.

Granted, actual pharmacy vitamins, such as Calcium, Vitamin D, B, etc. can be helpful for individuals who lack the proper amounts of these in their systems and have been recommended by a doctor.

Beauty supplements are only a fraction of the supergiant supplement industry that also includes detox teas and dietary supplements, join pain relief, inflammation, redness, etc. Be on the lookout and be smarter than the constant promotional messages telling you “it’s the best thing for (your problem)”. Sometimes you don’t even think that you have that problem, until you’re being advertsised to and convinced of it!
